LEAVE HULK ALONE!: At the beginning of your turn, if Hulk was targeted with an attack since your last turn, place an Anger token on his card. If Hulk wasn't targeted with an attack since your last turn, remove an Anger token from his card. After placing or removing Anger tokens, if Hulk has 5 Anger tokens or more he is immediately KO'd.

Does this trait only trigger once per turn, looking to see if Hulk was attacked at all, or does it trigger for each time that Hulk was attacked in the previous turn?

When the effect triggers and attempts to resolve, it checks to see if Hulk has been targeted with an attack since your last turn. The check is whether or not Hulk was targeted with an attack. The number of times he was targeted would not matter.
