Death and Grim Reaper have the following trait:

Harbinger of Death: When an opposing character takes damage from an attack made by a character with the Monster keyword, after resolutions, deal that character 1 unavoidable damage. If that character is not KO'd, immediately heal it 1 click.

If I have both of these guys on my force and the conditions for the effect are triggered, do I deal damage once for each of them?

The effect will trigger for each character on your force with the trait. Note that each triggered effect resolves separately. You will deal damage and then immediately heal and then repeat for each time it was triggered.
